Background
The gut microbiome promotes specific immune responses, and in turn, the immune system has a hand in shaping the microbiome. Cancer and autoimmune diseases are two major disease families that result from the contrasting manifestations of immune dysfunction. We hypothesized that the opposing immunological profiles between cancer and autoimmunity yield analogously inverted gut microbiome signatures. To test this, we conducted a systematic review and meta-analysis on gut microbiome signatures and their directionality in cancers and autoimmune conditions.

Methodology
We searched PubMed, Web of Science, and Embase to identify relevant articles to be included in this study. The study was conducted in accordance with the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) statements and PRISMA 2009 checklist. Study estimates were pooled by a generic inverse variance random-effects meta-analysis model. The relative abundance of microbiome features was converted to log fold change, and the standard error was calculated from the p-values, sample size, and fold change.

Results
We screened 3874 potentially relevant publications. A total of 82 eligible studies comprising 37 autoimmune and 45 cancer studies with 4208 healthy human controls and 5957 disease cases from 27 countries were included in this study. We identified a set of microbiome features that show consistent, opposite directionality between cancers and autoimmune diseases in multiple studies. Fusobacterium and Peptostreptococcus were the most consistently increased genera among the cancer cases which were found to be associated in a remarkable 13 (+0.5 log fold change in 5 studies) and 11 studies (+3.6 log fold change in 5 studies), respectively. Conversely, Bacteroides was the most prominent genus, which was found to be increased in 12 autoimmune studies (+0.2 log fold change in 6 studies) and decreased in six cancer studies (−0.3 log fold change in 4 studies). Sulfur-metabolism pathways were found to be the most frequent pathways among the member of cancer-increased genus and species.

Conclusions
The surprising reproducibility of these associations across studies and geographies suggests a shared underlying mechanism shaping the microbiome across cancers and autoimmune diseases.

Purpose of review: Time-restricted eating (TRE) entails consuming energy intake within a 4- to 10-h window, with the remaining time spent fasting. Although studies have reported health benefits from TRE, little is known about the impact of TRE on common chronic diseases such as type 2 diabetes, cancer and cardiovascular disease. This review summarizes and critically evaluates the most recent TRE research findings relevant to managing and treating these chronic diseases.

Recent findings: Most recent TRE studies have been in populations with overweight/obesity or metabolic syndrome; two have been in populations with diabetes, three in cancer survivors and none in populations with cardiovascular disease. Collectively, these studies showed that participants could adhere to TRE and TRE is well tolerated. These studies also showed preliminary efficacy for improved glucose regulation and insulin sensitivity, a reduction in body fat and blood pressure, reduced cardiovascular risk scores and increased quality of life. More research is required to define the most effective TRE protocol (i.e. length and timing of eating window, intervention duration).

Summary: TRE has demonstrated benefits on cardiovascular, metabolic and clinical outcomes relevant to the underlying pathophysiology, but there are limited data on TRE implemented specifically within populations with diabetes, cancer or cardiovascular disease.

Background and Purpose: Intravascular lymphoma (IVL) is characterized by the proliferation of lymphoma cells within small blood vessels. Diagnosis of IVL is difficult, and neurologic manifestations of the disease are common. Currently, there is little information related to the clinical course and rehabilitation of patients with IVL. This case report describes the presentation and treatment of an individual with IVL presenting as recurrent strokes.

Case Description: A 53-year-old man was admitted for treatment of bilateral multifocal infarcts in cerebral watershed areas. Initial assessments indicated that he required minimal assistance for functional activities, and he was discharged home with outpatient services. He was readmitted 7 days later with worsening confusion, left hemiparesis, and deteriorating functional mobility. Rehabilitation team concerns related to the patient's progressive functional decline led to additional testing, resulting in a diagnosis of IVL.

Outcomes: The patient initially required relatively little assistance with most functional activities, scoring 23 to 24/24 and 21/24 on the Activity Measure for Post-Acute Care (AM-PAC) Basic Mobility and Daily Activity Scales, respectively, 46/56 on the Berg Balance Scale (BBS), and 19/30 on the Montreal Cognitive Assessment (MoCA). He was subsequently readmitted and demonstrated a rapid functional decline, as evidenced by AM-PAC and BBS scores of 7/24 and 6/56, respectively.

Discussion: This patient presented with neurological deficits and an initial diagnosis of bilateral multifocal infarcts. Documentation of his progressive functional decline and decreasing AM-PAC and BBS scores facilitated interdisciplinary communication and led to additional diagnostic testing, resulting in a final diagnosis of IVL and appropriate medical intervention.

Objective: The objective of this study was to systematically evaluate the data regarding the use of neoadjuvant, perioperative, surgical, and adjuvant treatment options in localized gastric cancer patients and to develop Appropriate Use Criteria recommended by a panel of experts convened by the American Radium Society.

Methods: Preferred reporting items for systematic reviews and meta-analyses methodology was used to develop an extensive analysis of peer-reviewed phase 2/2R/3 trials, as well as meta-analyses found within the Ovid Medline database between 2010 and 2020. The expert panel then rated the appropriateness of various treatments in 5 representative clinical scenarios through a well-established consensus methodology (modified Delphi).

Results: For patients with medically operable locally advanced gastric cancer, the strongest recommendation was for perioperative chemotherapy based on high-quality data. Acceptable alternatives included surgery followed by either chemotherapy or concurrent chemoradiotherapy (CRT). For patients with upfront resection of stages I to III gastric cancer (no neoadjuvant therapy), the group strongly recommended adjuvant therapy with either chemotherapy alone or CRT, based on high-quality data. For patients with locally advanced disease who received preoperative chemotherapy without tumor regression, the group strongly recommended postoperative chemotherapy or postoperative CRT. Finally, for medically inoperable gastric cancer patients, there was moderate consensus recommending definitive concurrent CRT.

Conclusions: The addition of chemotherapy and/or radiation, either in the neoadjuvant, adjuvant, or perioperative setting, results in improved survival rates for patients compared with surgery alone. For inoperable patients, definitive CRT is a reasonable treatment option, though largely palliative.

Abstract: Despite the availability of prophylactic human papillomavirus (HPV) vaccines, there is a growing incidence of HPV-associated head and neck squamous cell carcinomas (HPV-HNSCC) worldwide. The viral etiology of HPV-HNSCC provides an opportunity to develop personalized immune-based therapies, which target the unique viral- or tumor-specific proteins. Novel HPV-targeted immunotherapeutic approaches in clinical development are reviewed. Early results from these trials highlight new opportunities and potential challenges ahead. Immunotherapies for HPV-associated HNSCCs will require a tailored combinatorial approach based on preexisting mechanisms of host immune resistance. As the field continues to identify the relevant HPV types 16 and 18 immunogenic epitopes that are presented by diverse HLA class I alleles, improved HPV-targeted biologics and clinical monitoring tools can be developed and applied to a broader cancer patient population.

Background: Fear of cancer recurrence (FCR) is one of the most common and aversive psychological phenomena among cancer survivors. Understanding the independent and potentially modifiable risk factors that contribute toward FCR seen in cancer survivors would inform future interventional trials aimed at reducing this risk.

Objective: The aim of this study was to provide an evidence synthesis of factors correlated with FCR to inform the development of preventive interventions.

Methods: A literature search was performed of the PsycINFO, EMBASE, and MEDLINE (PubMed) databases. Reports published from inception to 2020 focusing on the correlates of FCR with physical, psychological, and social factors were identified. Authors assessed the studies' risk of bias in accordance with the Standard Quality Assessment Criteria for Quantitative Studies (QualSyst criteria) and followed the Preferred Reporting Items for Systematic Reviews and Meta-Analyses statement. After extracting data and assessing the quality of the included studies, meta-analysis was conducted using RevMan 5.3 software.

Results: Thirty-four articles were included in this study with a total sample of more than 13 000 cancer patients. Fear of cancer recurrence was positively correlated with anxiety, depression, avoidance, chemotherapy, distress, intrusive thoughts, fatigue, rumination, and neuroticism. It was negatively correlated with optimism, age, social support, quality of life, time since diagnosis, well-being, and self-efficacy.

Conclusions: We identified 16 factors that are correlated with FCR, 9 positively correlated with FCR and 7 negatively correlated with FCR.

Implications for Practice: The findings of this study provide direction for the development of precise interventions for FCR in cancer survivors and lay the foundation for the further construction of an FCR-related nursing theoretical framework.

Purpose of review: Oral pain is a common complaint in patients with cancer. This review aims to summarize the knowledge on the causes and approach to management of oral pain garnered over the past 2 years.

Recent findings: A systematic review and meta-analysis included in the review, assessed cannabinoid versus placebo and showed only a small effect on pain, physical function, and sleep quality. Another review showed that chemical neurolysis as an adjunctive therapy, is effective in patients with pain of shorter chronicity and refractory head and neck cancer-related pain.

Summary: Patients with cancer frequently experience oral pain because of a variety of factors. Factors inherent in the type and location of the malignancy, the modality of cancer treatment, and a holistic approach to management together contribute to their overall pain experience. Basic oral care should be implemented wherever possible, before, during, and after cancer treatment.

Purpose of review: To summarize the current literature on the psychological impact of COVID-19 on patients with cancer, both in terms of the impact of the virus itself and of changes in the healthcare system; and to describe current recommendations for supporting patients with cancer during the pandemic.

Recent findings: Multiple studies have shown that patients with cancer experience high levels of psychological distress during COVID-19. Factors of greater vulnerability have been described as: being young, being female, low socioeconomic status, lower educational level, having low levels of hope or optimism, lower social support, and having cancer with curative intent. The severe acute respiratory syndrome-coronavirus-2pandemic has accelerated the healthcare digitization process. All departments involved in the diagnosis and treatment of cancer have made contingency plans to minimize the impact on patients.

Summary: Psychological distress is one of the most frequently occurring symptoms in patients with cancer during the pandemic. The COVID-19 pandemic has led to a restructuring of the healthcare system. The paradigm shift may pose a challenge for both healthcare professionals and patients.

Impact of COVID-19 on cancer screening: a global perspective | Septembre 2022 H

Purpose of review: Cancer screening disruption due to COVID-19 may have a significant impact on patients, healthcare practitioners, and healthcare systems. In this present review, we aim to offer a comprehensive view of the effect of the COVID-19 pandemic on cancer screening programs worldwide.

Recent findings: The present review comprised 33 publications. During the COVID-19 pandemic, the rates of cancer screening investigations were drastically reduced worldwide. Screening rates plummeted for all types of cancer, all ages, and racial/ethnic groups. Reductions in screening percentages were higher in several underserved racial groups. Cancer screening was suspended for at least 30 days in 13 countries.

Summary: Screenings for all types of cancer fell sharply. In the chain of cancer management, delays in any step are liable to change the outcome of the next step. Further long-term research will be needed to fully comprehend the impact of the pandemic on cancer services and patient outcomes.

Accompagner vers la mort son enfant atteint de cancer et faire le deuil de celui-ci représente une expérience bouleversante et souffrante à laquelle aucun parent ne souhaite être confronté. En Amérique du Nord, plusieurs dizaines de milliers d’enfants vivent avec le cancer (Snaman et al., 2020). Au Canada, 20 % des enfants atteints de cancer ne survivront pas à leur maladie (Société canadienne du cancer, 2021). Les parents de ces enfants sont leurs proches aidants principaux. Toutefois, le processus psychologique et social que traversent les parents ainsi que leurs besoins psychosociaux durant la fin de vie de leur enfant atteint de cancer sont peu documentés. Le présent article présente une synthèse des connaissances sur l’expérience des parents qui accompagnent leur enfant atteint de cancer vers les soins de fin de vie dans une perspective psychosociale. Cette recension permet, dans un premier temps, de caractériser la transition des soins curatifs vers les soins palliatifs d’un enfant atteint de cancer avancé. Dans un deuxième temps, elle décrit les principaux facteurs psychosociaux qui influencent le vécu des parents à travers la trajectoire de soin de l’enfant, de l’annonce de la maladie jusqu’à la période de deuil. Enfin, des recommandations pour améliorer le soutien psychosocial offert aux parents tout au long de la trajectoire de soin de l’enfant malade, incluant la phase palliative et celle du deuil, sont proposées.

Noninvasive respiratory supports for the relief of terminal breathlessness | Juin 2022 H

Purpose of review: Breathlessness is a common symptom in patients with respiratory failure in the terminal phase of their illness. Noninvasive methods of oxygen delivery are frequently used in the palliative setting. We review the evidence supporting noninvasive respiratory supports for the relief of terminal breathlessness in those with life-limiting illnesses.

Recent findings: There is limited evidence to support the use of supplemental oxygen for patients without hypoxia. It is unclear whether the symptomatic benefit of oxygen therapy relates to the oxygen delivery and/or airflow across the nasal mucosa. Early trials suggest that high-flow nasal cannula (HFNC) oxygen therapy improves breathlessness at rest and on exertion for patients with cancer. Noninvasive ventilation (NIV) also appears to improve breathlessness in the palliative setting; however, potential harms include facial pressure injuries, claustrophobia and anxiety. Goals of care should be explicitly discussed and frequently reviewed given that these interventions have the potential for harm and can be challenging to withdraw.

Summary: HFNC oxygen therapy and NIV appear to reduce breathlessness in the palliative setting. Further high-quality trials are needed to confirm the symptomatic benefits of noninvasive respiratory supports on breathlessness for patients with cancer.

Background
There is limited information on the risk of hospital-acquired coronavirus disease 2019 (COVID-19) among high-risk hospitalized patients after exposure to an infected patient or healthcare worker (HCW) in a nonoutbreak setting.

Methods
This study was conducted at a tertiary care cancer center in New York City from 10 March 2020 until 28 February 2021. In early April 2020, the study institution implemented universal severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) testing at admission and retesting every 3 days through the hospital stay. Contact tracing records were reviewed for all exposures to SARS-CoV-2 positive patients and HCWs.

Results
From 10 March 2020 to 28 February 2021, 11 348 unique patients who were SARS-CoV-2 polymerase chain reaction (PCR) negative at the time of admission underwent 31 662 postadmission tests during their hospitalization, and 112 tested positive (0.98%). Among these, 49 patients housed in semiprivate rooms during admission resulted in 74 close contacts and 14 secondary infections within 14 days, for an overall attack rate of 18.9%. Among those exposed to a roommate undergoing an aerosol-generating procedure (AGP), the attack rate was 35.7%. Whole genome sequencing (WGS) corroborated transmission in 6/8 evaluated pairs. In addition, three transmission events occurred in 214 patients with significant exposure to 105 COVID-19 positive healthcare workers (1.4%).

Conclusions
The overall risk of hospital-acquired COVID-19 is low for hospitalized cancer patients, even during periods of high community prevalence. However, shared occupancy with an unrecognized case is associated with a high secondary attack rate in exposed roommates.

Opioid use disorders and associated morbidity and mortality have increased with dramatic rise during the SARS-CoV-2 pandemic. National and international stakeholders have developed clinical practice guidelines in an effort to curb opioid misuse and overdose-related death. However, to ensure that patients with cancer do not experience barriers to adequate pain management, most of these guidelines are not intended for patients with cancer-related pain or for those receiving palliative or hospice care. Oncology, palliative, and hospice care providers are increasingly tasked with the management of severe disease-related pain in the setting of coexisting opioid use disorder without research on the most effective risk and harm reduction strategies to guide care.

Clinicians should be familiar with addiction medicine and chronic pain literature and be able to incorporate some of these best practices. This case study reviews the management of severe cancer-related pain in a patient with co-occurring opioid use disorder, utilizing many of the best practices in available clinical practice guidelines for the management of chronic non-cancer-related pain.

Purpose of review: Early palliative care (EPC) is known to generally improve both health-related quality of life (QoL) and symptom intensity at small effect sizes. However, it is unclear whether EPC is effective in older people, a population that is notoriously unaccounted for. This review summarizes the recent evidence concerning the efficacy of EPC in older patients with advanced cancer and delineates existing barriers to accessing respective services.

Recent findings: The search for studies published in MEDLINE from January 2020 to September 2021 yielded six relevant records. Data from a recent feasibility trial and subgroups from larger randomised trials point to a somewhat lesser decline in QoL for patients undergoing EPC compared to those receiving treatment as usual. However, enrolling older patients in such trials remains a major challenge mostly due to them feeling too ill to participate.

Summary: For older patients, the efficacy of EPC, like many other medical interventions, has hardly been studied so far. Existing work yielded several specific barriers for older patients to access this type of care. Future research should prioritize efficacy trials of EPC tailored to the needs of older patients enabling clinicians to enter truly evidence-based shared decision-making with their patients.

Purpose of review: The aim of this study was to summarize the literature on the impact of COVID-19 on older adults with cancer, including both the impacts of COVID-19 diagnosis on older adults with cancer and the implications of the pandemic on cancer care via a synthesis of reviews, guidelines and other relevant literature.

Recent findings: Our synthesis of systematic reviews demonstrates that older adults with cancer are prone to greater morbidity and mortality when experiencing concurrent COVID-19 infection. Current evidence related to the association between anticancer treatment and COVID-19 prognosis for older adults with cancer is conflicting. Guidelines and recommendations advocate for preventive measures against COVID-19; the uptake of telemedicine and virtual care; encourage vaccination for older adults with cancer; and the use of geriatric assessment.

Summary: The COVID-19 virus itself may be particularly deleterious for older adults with cancer. However, the health system and social impact of the pandemic, including global disruptions to the healthcare system and related impacts to the delivery of cancer care services, have equally important consequences.

Objective
Physical therapist-delivered rehabilitation aims to manage the side effects of cancer and its treatments. Although access to cancer rehabilitation is not yet a standard of care in many countries, physical therapists practice in many types of cancer services with different cancer populations. The purpose of this study was to explore the experiences of physical therapists in cancer care practice with regard to their role, the factors influencing service delivery and development, and physical therapists’ professional development needs.

Methods
In this qualitative study with semistructured interviews, physical therapists in cancer care settings in the Republic of Ireland were interviewed via telephone. Participants (n = 17) represented a variety of clinical settings and roles. Two researchers performed thematic analysis of transcriptions using a semantic, inductive approach. Key themes and codes were identified and illustrative quotes were selected.

Results
Six main themes were found: the need for more services, barriers to service development and delivery, a lack of awareness of the role of physical therapy, facilitators to service development, goals for the future of oncology physical therapy, and training needs of staff.

Conclusions
Physical therapists provide valuable interventions across the spectrum of cancer care but experience barriers to the delivery and development of services. Investment in oncology physical therapy and developing international standards of care will allow physical therapists to meet the rehabilitation needs of survivors of cancer.

Impact
As international guidelines increasingly recommend development of cancer rehabilitation programs, it is important to understand physical therapists’ experiences of working in cancer care to assist in the development of effective oncology physical therapy services. This study demonstrates that physical therapist–led cancer rehabilitation services need investment and public promotion to enable the provision of optimal services to all patients with cancer and to meet standards of care.

Le présent article est le troisième d’une série intitulée « Prévenir la thrombose », qui vise à mieux faire connaître la thrombose associée au cancer (TAC) et ainsi améliorer les connaissances des malades et des soignants à ce sujet, l’état des patients et, en bout de ligne, réduire le fardeau de cette comorbidité. Les deux premiers articles, Importance de la thromboembolie veineuse liée au cancer (Sardo et al., 2021) et Ce que les personnes atteintes de cancer veulent savoir (Bayadinova et al., 2022) ont décrit la TAC, le manque de connaissances sur cette affection touchant la population atteinte de cancer, puis présenté des stratégies de sensibilisation et d’éducation destinées aux malades. L’objectif de ce troisième article est de sensibiliser les professionnels de la santé à la thrombose liée au cancer, en dégageant les lacunes relatives au savoir du personnel médical, en suggérant des outils pour identifier les patients à plus haut risque et en proposant des stratégies et des ressources pour sensibiliser davantage le personnel soignant.

La collecte des antécédents familiaux de cancer permet d’évaluer la prédisposition à certains types de cancers héréditaires. Une enquête par questionnaire a été menée auprès d’infirmières travaillant auprès de personnes atteintes de cancer afin d’évaluer leurs connaissances en oncogénétique, de décrire les enjeux liés à leur capacité à identifier, à référer et à accompagner des personnes présentant un risque héréditaire de cancer et d’explorer leur intérêt pour de la formation continue. Les résultats démontrent des connaissances et un sentiment de compétence retreints chez les infirmières et un accès limité à de la formation universitaire et continue. De la formation axée sur le développement de leurs compétences renforcerait leur capacité à procéder à l’évaluation initiale des personnes potentiellement à risque de cancer et à les orienter vers des ressources spécialisées.

Objectif : La fatigue et les troubles cognitifs qui se manifestent après un cancer sont des effets indésirables qui, souvent, perdurent après les traitements et nuisent à la qualité de vie. L’étude vise à évaluer la faisabilité du neurofeedback et ses effets sur le fonctionnement cognitif et la fatigue des survivants du cancer, particulièrement la faisabilité des stratégies de recrutement, du protocole de recherche et des mesures de résultats.

Devis : L’étude pilote de faisabilité est construite selon un devis avec groupe témoin mis sur liste d’attente pendant une période de 10 semaines. Les participantes servaient elles-mêmes de groupe témoin. Elles ont suivi deux séances de neurofeedback par semaine, pendant 10 semaines.

Participants : L’échantillon était composé de survivantes du cancer du sein vivant à Kingston, en Ontario (n=16).

Méthodologie : Les résultats ont été évalués à l’aide d’échelles d’autoévaluation validées, ainsi que par des tests neurobiologiques avant, pendant et après le traitement par neurofeedback.

Résultats : La faisabilité du protocole de neurofeedback a été confirmée : la procédure s’accompagne d’une diminution significative des troubles cognitifs, de la fatigue, des troubles du sommeil et des symptômes psychologiques.

Implications pour les services psychosociaux : Le neurofeedback pourrait se poser en thérapie complémentaire efficace et non invasive pour traiter les troubles cognitifs qui surviennent chez les survivantes du cancer du sein après les traitements.
